Registration fees are based on the date of receipt of the Group Registration Form and payment.
The prices indicated are ONLY applicable if BOTH the Group Registration Form AND the payment are received before the deadlines, otherwise the higher registration fee will be applied.
The registration fee includes: attendance to the all scientific sessions, entrance to the exhibit and poster area, final program, abstract book, attendance certificate, badge and congress bag, the lunches and coffee breaks.

Duly complete and scan the hereby Group Registration Form Download and save on the hard disk of your computer the Excel sheet called “Reporting Group - MSDA 2012” available below Enter the list of your invited delegates in this Excel sheet. Please fill in each field and save your records E-mail the duly completed and scanned Group Registration Form together with the Excel sheet as attached documents to the MSDA 2012 Organizing Secretariat at: msda@agence-plb.com The MSDA 2012 Organizing Secretariat will acknowledge receipt of your documents and will keep you posted of the next steps |

Please note that no confirmation will be sent to your invited delegates.
This is the Group Contact’s and/or the Group’s Corporate Sponsor’s responsibility to confirm the registrations to his/her invited delegates.
An on-site group representative is required for your group. If an on-site group representative is not designated, then your participants will be considered as individuals which means that they will have to collect their conference kits individually at the Congress Welcome Desk.
The on-site group representative is responsible for the pick-up and distribution of conference badges and materials to group members.
The group representative will be contacted by the Organizing Secretariat to set an appointment for Tuesday, May 15th , 2012 from 2.00 pm to 5.00 pm and Wednesday. No appointments will be available on any other dates. At that appointment the group representative will pick up badges and materials for all group members and will also have the opportunity to address changes, cancellations, etc…
Any cancellation should be notified in writing to the Organizing Secretariat by email at:
msda@agence-plb.com and will be subject to the following conditions:
Before March 31st, 2012, refund of registrations subject to a 30% administrative charge.
All refunds, if any, will be made after the congress
After March 31st , 2012 no refund
Replacements of confirmed registration are accepted with a charge of 50 € per participant until April 30th, 2012. Beyond no refund.
All registration fees are quoted in Euro (€). Payment in any other currency will not be accepted neither for advance registration nor for on-site registration.
It is essential that a copy of the bank transfer details and a copy of the official completed Group Registration Form are faxed or scanned and e-mailed to the Organizing Secretariat in order to identify your bank transfer. The bank transfer must clearly state the name of the group’s corporate sponsor and/or the name of the company which manages your group, as unidentified bank transfers cannot be processed.
A bank cheque in Euro made payable to “PLB. Organisation Inc./MSDA 2012”
Please note that American Express, Dinners cards will not be accepted.
Please enter all relevant information on the Group Registration Form and sign for authorization. It is also imperative that the credit card holder name is clearly mentioned if it differs from the group contact name.
